Thank you for participating in Quincy Humane Society’s Volunteer Training.
We look forward to having you join our team to help love and care for our animals.

Below is our volunteer acknowledgement and waiver form.

Quincy Humane Society Volunteer Acknowledgement and Waiver

Once volunteers have completed a training walkthrough, we ask that all volunteers fill out and submit the waiver and liability form.

I agree to conduct myself in a courteous and professional manner as a volunteer and representative of the Quincy Humane Society. Displaying behavior not representative of the Quincy Humane Society and its policies will result in immediate removal from the volunteer program.

I agree to abide by all of the Quincy Humane Society policies and procedures. Failure to comply with shelter rules will result in immediate removal from the volunteer program.

I agree that my services are provided to the Quincy Humane Society on a volunteer basis without pay or compenation of any kind and all services are performed at my own risk.

I hereby allow the Quincy Humane Society to use any photographs taken of me on proerty or at a specialevent for public relations purposes.

I currently have active health insurance.

I fully understand that the activities associated with volunteering, although unlikely, involve risks and dangers of bodily injury, including permanent disability, paralysis, and death. I understand that these rsks and dangers may be caused by my own actions or inactions, the actions or inactions of others, the condition in which the various activities take place, and/or the negligence of the Quincy Humane Society. I also understand that there may be other risks and dangers, social and economic lossesI may incur as a result of my participation.

I on behalf of myslf and minor child (if applicable) release, discharge and hold harmless the Quincy Humane Society, Inc. together wth any of their respective administrators, directos, agents, officers, members, volunteers, employees, other participants, sponsors, advertisers, and if applicable, owner and lessors of premises on which volunteer activities take place (hereinafter referred to as "released parties) from all liability, claims, demands, losses or damges on my account caused or alleged to be caused in whole or in part by the negligence of the released parties. I further agree that, if despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Released parties I will indemnify, save, and hold harmless each of the released parties from any litigation expenses, attorney fees, loss, liability, damage, or cost which they may incur as a result of such claim.
